• Athletics and General Programs Mail-In Registration Form (PDF)
• Adult League Player Contract (PDF) - This form must be completed and returned to the Athletic Program Supervisor before you are eligible to participate in any adult league game.
• Emergency Information Form (PDF) - For summer camps
• Permission to Pickup Form (PDF) - For summer camps
• Volunteer Application (PDF) - This is a form identifying interests and skills of those wishing to volunteer time to the Recreation, Parks, & Cultural Resources Department.
• Century Center Guideline Form (PDF) - This forms give basic information and rental information for the Carrboro Century Century activity rooms and Century Hall.
• Services Proposal Application (PDF) - This application is used, by an instructor, to propose a new program or activity to the Recreation, Parks, & Cultural Resources Department.
• Financial Assistance Application (PDF) - This application is used to apply for a fee reduction or waiver when registering for a program or class.
• Refund Request Form (PDF) - Requests for registration refunds or park facility refunds must be submitted using the Refund Request Form; the form may be faxed to 919-918-4475 or it can be mailed to:
